Please note that Jaya Life does not provide emergency services outside of opening hours. Therefore, we recommend that all patients maintain their registration with their NHS GP practice.

Booking appointments:

Appointments can be booked online or via telephone. Confirmation will be sent by email with patient consent.

Referrals:

In the event that a patient requires a referral to a specialist service, the reasons and options for facilitating the referral will be explained by our clinicians and support and guidance will be provided throughout the process.

Payment:

A full list of fees for all our services is available on our website, www.jayalife.com

If any additional services are required following an initial consultation, the patient will be informed of any extra charges. Such services will only be provided with the patients full informed consent.

We accept Credit/Debit card, BACS and cash payment.

Confidentiality:

The clinic needs to collect personal information about people with whom it deals in order to carry out its business and provide its services

The lawful and proper treatment of personal information by Jaya Life is extremely important to the success of our business and in order to maintain the confidence of our service users and employees. We ensure that the clinic treats personal information lawfully and correctly.

We support fully and comply with the principles of the Act which are summarised below:

  • Personal data shall be processed fairly and lawfully.

  • Personal data shall be obtained/processed for specific lawful purposes and will only be used for the purpose for which it was collected.

  • Personal data held must be adequate, relevant and not excessive.

  • Personal data must be accurate and kept up to date, and every reasonable step will be taken to ensure any personal data that is inaccurate is erased or rectified without delay.

  • Personal data shall not be kept for longer than necessary.

  • Personal data shall be processed in a manner that ensures appropriate security of the personal data.
